DELAY IN DIAGNOSIS OF CANCER

A woman found a small lump by her ear. Her treating doctor did the right thing and ordered a CT scan to evaluate it. Unfortunately, the radiologist, who saw the mass on the CT scan concluded it was a “normal variant” and that no further testing or biopsy was required. When the mass grew larger, the patient went back to a different doctor who made the correct diagnosis of cancer. Unfortunately by that time, it was too late as the cancer had progressed from a curable Stage 1 to an incurable Stage 4. The patient now has only two to three years to live.
The family received  a $3.38 million verdict, but it will be reduced by the damage cap.  “Nothing’s ever going to be the same for them,” the lawyer said. “Money can’t change that, but it can help ease the suffering somewhat.” After the verdict, her lawyer also said the message is that for patient safety, doctors should “err on the side of ordering the next test.”

Failures in the new electronic medical records system has harmed almost 150 veterans

The rollout of the Electronic Health Record system for the VA has been described as “totally unacceptable,” “with real patient safety risks that have gone unresolved for entirely too long.” According to this report in the Military Times, the VA recently suspended the rollout of the EHR system at four of its pilot sites after the VA concluded the software was not performing correctly and after the VA inspector general concluded that nearly 150 veterans had been harmed because of failures in the electronic records system.

Why has the United States not reduced stillbirths?

As many as 25% of stillbirths may be preventable.

 

According to the CDC, with around 21,000 deaths in 2020, stillbirths claimed more children’s lives than car accidents, drowning, fire, flu, guns, listeria, poison, and sudden infant death syndrome combined. A 2018 study published by Obstetrics and Gynecology concluded that almost 25% of stillbirths may be preventable. As many as half of women detect a decrease in movement and kicks before a stillbirth, but one doctor described education on fetal movement and stillbirth prevention as a “massive hole” in obstetrics. Some experts also think such a decrease should be taken more seriously by health care providers.

 

Despite the shocking statistics, the U.S. is the only wealthy country that doesn’t have a national system to report and investigate stillbirths, although newly-introduced legislation may help. In March 2022, the Stillbirth Health Improvement and Education for Autumn Act, or SHINE was introduced in the Senate. If passed, the legislation — which previously passed in the House in December 2021 — would authorize grants to develop programs that may help save some of these babies.

Countries that have focused on stillbirth education and prevention have better outcomes. In 2019 in Australia, some maternity care providers implemented prevention programs and reported a 21 percent reduction in stillbirths. The United Kingdom launched a similar program and in just 2 years saw a 20 percent drop in stillbirths. What can you do? While many stillbirths can’t be prevented, for those cases where something can be done – make sure to discuss tracking fetal movements with your doctor, do your kick counts, and make sure your doctor checks out your baby carefully if you report decreased movements!

Doctors are less likely to respect patients who are overweight or obese

Research has long shown that doctors are less likely to respect patients who are overweight or obese.  This may result in blaming all of a patient’s problems on obesity and telling them to simply eat less and exercise more, without doing a careful investigation to find out the true cause of a health concern.  “This is almost like malpractice. You have these physicians or clinicians — whoever they are — relating everything to the patient’s obesity without investigation,” one expert explained in a recent article, below.

To counter that, in June the Association of American Medical Colleges plans to roll out new diversity, equity, and inclusion standards aimed at teaching new doctors, among other things, about respectful treatment of people diagnosed as overweight or obese.  Hopefully, there will be some education of all currently practicing doctors as well.
